What Options are Available for Assisted Living?
In the state of North Carolina an assisted living residence provides group housing, allowing nursing services on a case-by-case basis. There are two types of facilities: adult care homes and multi-unit assisted housing with services (MAHS). Adult care homes can choose to serve individuals 55 years old and older or those with Alzheimer’s and other cognitive impairments. MAHS provides hands-on personal care services and nursing services are provided by a licensed home care or hospice agency, through an individualized written care plan.
Adult care homes are required to comply with state and local fire safety regulations, utilizing some detectors in all corridors with no more than 60 feet between each and no more than 30 feet from any end wall. These detectors must be placed in all storage rooms, kitchens, living rooms, dining rooms, and laundries, and they must be interconnected with the alarm system in place.

What Training is Required for Assisted Living Staff?
Direct care staff in adult care homes must complete 80 hours of training within the first 6 months of employment.
Special care staff must complete 6 hours of orientation within the first week of employment, 20 hours of dementia-specific training within six months of employment, and 12 hours of additional training each year.
Is an Assessment Required Before Move-In?
MAHS screen prospective residents to help determine the facilities ability to meet the resident’s needs and to determine the amount of licensed home health assistance they will require. In an adult care home an assessment is required within the first 72 hours of residency, but must be completed within the first 30 days following admission. It must also be updated annually.
What Care is Available in an Assisted Living Residence?

In both types of facilities hospice and home health care may be requested by the resident and provided with the appropriate physician orders. IN a MAHS, personal care and nursing services are provided through licensed agencies.
How is Medication Managed?
Self-administration of medications is allowed in a MAHS provided the staff has been adequately trained and are under the delegation of a licensed nurse. Adult care homes are required to administer medications - the staff must be validated by a registered nurse (RM) and pass a written exam offered by the state.
What are the Bathroom Requirements for an Assisted Living Residence?
Bathrooms may be shared in adult care homes, requiring 1 toilet and sink for every 5 residents, and 1 bathtub or shower for every 10 residents.
What are the Move-In or Move-Out Requirements?
MAHS can only admit residents whose needs can be met by the facility, and they may not admit or retain residents who:
- Require 24-hour supervision unless a physician certified that appropriate care can be provided.
- Are dependent on a ventilator.
- Have stage III and IV ulcers.
- Require intravenous therapy or injections.
- Have an airborne infectious disease in a state that requires isolation or special precautions.
- Take psychotropic medications without an appropriate diagnosis and treatment plan.
- Nasogastric or gastric tubes (unless he or she is able to fully care for them themself).
- Requiring assistance with 4 or more activities of daily living (ADLs).
An adult care home is more mentally and physically dependent where 24 hour supervision and assistance with scheduled and unscheduled personal needs are required, so in additional to the above points, they may not admit nor retain residents who:
- Require treatment for mental illness or alcohol or drug abuse.
- Require maternity care.
- Require nursing under continuous medical supervision.
- Pose a direct threat to the health and safety of others.
Is Assisted Living Covered by Medicaid in the State of North Carolina?
Yes. A state plan service through Medicaid covers personal care services in an adult care home.
