A LPN and/or RN would complete an assessment. Quarterly the assessments are completed. However, the individual service plan is a working tool we can update if residents needs and or expectation have changed.

We have monthly in-services that the caregivers complete online. We also have monthly in-services with department heads training.

Monthly in-services and training online. We partner with dementia specials quarterly to provide trainings to our staff members.

We have a system called variable staffing - we staff according to our care needs. We have 24-7 nursing.

WE are NOT affiliated with a hospital. Beaumont Troy is the closest hospital to our location. Families and residents still have the opportunity to pick what provider they would like to have loved ones sent to.

We have a secured memory care community. If someone is a wander risk, they would blossom in our memory care neighborhood.

State of the art pendant system. Resident has pull codes in ever room of there apartment. Joyce of watch or necklace pendent to wear on their person. Several room check throughout the day.

We have special diets available. Puree aid, mechanical soft and thicken liquids. We have feed assistance available.

Troy Senior Center is across the street. Dog Park within walking distance. Big Beaver is 1/2 mile away. Great dinning and Shopping for both the residents and their family members.

Open Style dining from 7 am-7 pm. Restaurant-style with a full menu available. Fresh food daily. Steak, chicken, and seafood are options on the menu. Homemade soups daily.

Anthology of Troy offers transportation to medical appointments, personal errands and fun outings with our engagement team.

We are a PET Friendly community. We also have therapeutic pet visits monthly at our wonderful community. We have lots of beautiful garden area for walking the dog. Plus, the dog park is right next door.

Our team knows how important it is to stay in communication with our families. We have an open-door policy, we will always take the time to speak with you, answer an email or pick up the phone. Our nurse's department will provide individual service plans upon move in - this is an ongoing working tool to communicate needs and services. We would have a meeting to communicate changes in the plan if no changes in a quarterly meeting. Communicate the doctor's appointments and medication changes. Sagely is a wonderful app provides to families to let them know what activities their loved ones participated in weekly.
