Care staff are 1 to 8 during the day and afternoons. There is full time RN. Also, dietary staff on site, receptionist, Regional Operations Manager, Director of Sales, full time Activity Director and part time activity assistant.
Oak Park Place Oak Creek is in a residential and business zoned area - near 20th and Rawson Ave near I-94.
Doors in autumn Lane Memory Care Neighborhood are secured. Alarm beeps in event door bar is pushed in and opened. Audible alert plus message sent to every caregivers work phone.
At the time of hire, staff receives education on topics of understanding dementia, fire safety, universal precautions, First Aid, resident rights, patient confidentiality, etc.
We offer morning seated exercises, brain games, bingo, socials, arboretum gardening and entertainers.
Oak Park Place offers in-house visiting medical service called Bluestone medical. Also podiatrist visits every 61 days as covered by Medicare. We can bring in Medicare Home CAre RN, PT, OT and ST services.
Assisted Living population is nice mix of men and women - Average ages are in upper 80's. We celebrate many 90th birthdays here. Favorite activities are: socials, crafts, entertainers, bingo, indoor plant arboretum.
We have Regional Chef Kevin Thao and on-site Chef Chris Connor. Both have extensive culinary experience and are culinary school graduates. We try as much as possible to make food from scratch.
4 Facebook posts per week, Google reviews, monthly newsletter called The Acorn, weekly emails to families about COVID updates. Families welcome to call 24/7 for updates. Info released to power of attorney.
We welcome families to call in anytime 24 hrs per day where the Power of attorney can garner updated info on how mom or dad are doing. Activity staff use IPADS for virtual visits. We send out email info with COVID updates.
Staffing is higher in memory care neighborhood. Caregivers help feed residents and help them wear food protectors. Culinary staff can make mechanical soft, no added sugar diets etc.
Regular on-going training in dementia. Recently, video tapes narrated by Dementia Expert Teepa Snow were presented.
Ratio is 1 to 8 for daytime and afternoons. Night shift ratio 1 to 11.
We offer a minivan that has a wheelchair dock. We use a minivan after COVID restrictions lift.
Beautiful small park across Rawson Ave. Sendiks and other shopping within 1 mile west of Oak Park Place.
Activity Staff use wide variety of programming: one-on-one activities in their apartments, bingo , seated exercise, indoor gardening arboretum, crafts, memory games.
We have receptionist main business hours. After hours front doors are locked. Guests can call phone number posted inside first door to reach staff member to come and greet them and unlock door.
