At Sylvan Crossings we dedicate ourselves to providing quality care to all our residents in a trusting, secure, and respectable home-like environment. Our residents thrive in the social and active community we promote in each of our homes.
Our mission is to provide compassionate care in a warm, friendly environment while meeting the health, social, and individual needs of each resident. We pride ourselves on the strong relationships formed between our staff, residents, resident families, and the community.
When a resident chooses to make Sylvan Crossings their home, they meet with an interdisciplinary team that includes their families. Together we assess the services that are needed.
Residents are encouraged to bring their own furniture so they feel more at home. Each suite is spacious, has a 24-hour monitoring system, a full bath, individual thermostat, and an emergency call system.
Most residents appreciate routine assistance with personal care, such as bathing, medication monitoring, and dressing. Residents with walkers or wheelchairs enjoy mobility assistance. Incontinence care promotes independence and helps retain dignity. Optional services include on-site hair and nail care and assistance with transportation to offsite locations.
Menus for meals, made from scratch, are thoughtfully planned by our staff dietician. Residents have three meals in the dining room, and snacks are available all day long. A private dining room can be arranged for visiting family and friends.
We manage medications, including ordering and dispensing. Nurses provide regular consultations and assessments and regularly review medications.
Daily exercise is offered both for individuals and groups. Recreational programs such as games, crafts, and music are provided. We offer assistance with writing correspondence and reading. Private areas are available for family gatherings.

FAQ About Manor Lake Assisted Living & Memory Care
Hunter Ridge has three staff that are scheduled during daytime hours. During nighttime hours there are two staff scheduled.
Here at Hunter Ridge we offer a wide variety of things the residents can participate in. There are many for our residents to choose from such as; card games, bingo, trivia, exercise, spa days, and the favorite, bowling.
Hunter Ridge has an exceptional cook that has been with us for over 6 years. The menu varies based on what our Residents are craving/wanting. We hold a town meeting monthly to ensure our residents are receiving what they are wanting to eat.
Families of Hunter Ridge residents are encouraged to visit as often as they would like. Families of the residents will be updated with any changes and concerns that come about. Families can also call us anytime they would like to check in.
Hunter Ridge offers armed exit doors and alarmed front door. Residents with the need of safety measures are also on hourly or half hourly checks based on the need.
Our community is alarmed.
Staff receives monthly updated training required by the state all staff has completed the state-required training before working with residents.
We offer the community transit for the area. If the area transit is not available there is Stat Medical or Van Go that could be used. Beaver Dam transit is our preferred transpiration service.
There are a wide area of ages at Hunter Ridge. Our youngest resident is 61 and our oldest resident is 101. One of the most favorited thing to do here at Hunter Ridge is Bowling. We have bowling every Wednesday. The winner of bowling that day gets to keep the bowling pin until the following Wednesday.
They can enjoy the Resident's rooms or there is a spacious back and side yard to walk them.
Hunter Ridge has a spacious back yard. Our residents enjoy watching the wild life that visits. There is also a small creek that runs along the property that the ducks and geese frequent.
