Provision Living of Forest Hills has a diverse and dynamic resident population. Residents come from all walks of life from geologists to teachers to bankers and nurses, professors to housewives and military spouses. These diverse backgrounds give our community a rich and robust group of people who quickly become friends and find common ground. Residents range in age from 55 and older. Favorite activities include shopping, outings to surrounding parks and attractions. Our residents enjoy gathering for piano music from guests who frequently come in to play our baby grand piano in our front living room.
We have a well-rounded social and activities program that includes outings to area state parks, local museums and shopping. Local author book readings, pet visits, spiritual and cultural programs that enhance daily lives. Our community is located near the many colleges and university’s including, Grand Valley State University, Calvin College and Aquinas and benefits for the dynamic interaction with students from all over the world. We also have the Forest Hills Fine Art Center, aquatic and senior center right in our back yard as well as the YMCA – Mary Free Bed campus just a few miles away.
Provision Living of Forest Hills is located in an incredible area, right next to the Forest Hills Fine Arts Center, reflecting the communities spirited dedication to the arts. Behind out community lies our senior center, which provides forums, presentations and luncheons all within walking distance. In addition to that we have many shops and amazing restaurants just adjacent to our community. There are also paved trails that go around the local community surrounding us.
Provision Living of Forest Hills provides complimentary transportation via our wheelchair lift van to and from medical appointments. We also use our van to take our residents on outings to local attractions, shopping, restaurants, and entertainment venues.
Our Executive Chef has years of food and culinary expertise in fine dining and restaurants. He brings creativity and flavor to our menus and adds special touches and dishes that may be new to our resident’s palates. Our restaurant style meal service is delivered in our beautiful dining room that looks out over a patio and green space. We offer a featured meal each day for all the meals throughout the day, as well as an extensive ala carte menu. Ala carte selections are prepared to order at each meal.
Provision Living of Forest Hills is designed with a fully integrated alarm system that includes smoke detectors in all resident apartments & throughout the community. Our community is fully-sprinklered ensuring even greater safety for residents. Residents are provided with an emergency call system that is monitored in a central location & alerts staff when a resident needs assistance. All entry & exit doors are alarmed & provide a greater measure of security. Our front entrance is locked after 7 p.m. & requires a code & staff assistance to enter after hours. Guest, residents & vendors will sign in & out of the building with a program called Accushield. We are also monitored 24/7 via security video cameras to ensure the safest possible setting for each of our residents. In addition, we offer an incredible technology that is equipped in each room called Foresite, which is a predictive analytic system that monitors for falls & other diagnose such as CHF, COPD, UTIs & even pneumonia.
Provision Living of Forest Hills begins building a great communication & information exchange with residents & their families beginning with each resident’s pre-admission process. Our team spends time learning about each resident’s individual needs & preferences to ensure a person-centered, individualized plan using resident & family input. This plan is reviewed on regular basis & as resident’s needs or preferences change & update to reflect changes. We encourage our families to ask questions, visit with our leadership team & staff whenever possible to ensure we are sharing information effectively. Family members are invited to our Family Council meeting where they can communicate with the community leaders & share their experiences with other family members. Our full-time activities director shares photos with families of our residents engaged in our social & recreational activities programs through our Provision Living of Forest Hills Facebook page!
Provision Living of Forest Hills ensures that our caregiver-to-resident ratio meets or exceeds required State of Michigan requirements. Our ultimate goal is to ensure that each resident receives the care and supportive services they need to maximize their independence while living in our community.
Provision Living of Forest Hills is a pet-friendly community and we understand that our residents’ pets are family. Dogs and cats are welcome; however, must remain inside the resident’s personal apartment. Unfortunately, not everyone enjoys having pets in their lives and we must respect their rights as well as those of our pet owners. All pets must be leashed when outside the resident’s apartment, in common area, or outside on the grounds. We request that each resident and/or a family member ensure the care of the pet. Pets are not allowed in the dining rooms at any time.
Provision Living of Forest Hills is committed to ensuring that our caregivers are provided with ongoing education and training. Upon becoming a member of our team, each staff member is provided with an extensive orientation process that includes both State required training and additional training required by our community. Both new hire training and our ongoing continuing education programs include topics related to Dementia, Fire Safety and Emergency Preparedness, Resident Safety, Confidentiality and Safeguarding of resident information, Safe Medication Administration Techniques, and Customer Service. Training is provided for staff through in-person in-services and on-line continuing education.
Resident safety and security are a priority for the Provision Living of Forest Hills team. Safety measures include Sign-in and Sign-out logs via Accushield at the front entry. All entry and exit doors are alarmed and those alarms are linked to staff pagers. Video security cameras are placed throughout the community and monitored for activity. The front entry doors are locked from 7 p.m.- 8 a.m. daily to provide an additional measure of safety and security. All other doors always remain locked from the outside and require a key code or staff assistance to gain entrance. Residents with a known history of wandering are referred to the care of our Memory Care Neighborhood.
