Apple Creek Place's mission is to provide high-quality care to our residents in a warm and safe environment. Our caring staff will work together with your physician, as a team consisting of experienced nurses, caregivers, and administrators to provide you or your loved one with the best possible care. We have 24-hour nursing care staff, a beauty shop, daily activities, a relaxing sunroom, free wifi internet, and our own personal chef. Family and friends are more than welcome to visit. We have spacious parking and an annual event picnic for family time. Here at Apple Creek Place, we make it our goal to provide you with a place you can call "your home" where you can live comfortably, with pride and dignity through the rest of your life.

Apple Creek Place works with multiple care providers in the area, allowing resident's the freedom to choose where they would like to receive additional care when needed.
Apple Creek Place employs a full-time Life Enrichment Coordinator that provides meaningful activities to residents based on their needs and interests. Activities include: morning stretches, memory games, baking club, movie nights, tailgate parties, bingo, Wii sports games, bean bag toss, music therapy, trivia contests,
Assessments are completed at least 30 days prior to move-in, 30 days after move-in and annually by our staff RN/Health Care Coordinator. Assessments are also done as-needed after a major change in condition, or at the request of family or guardians.
Our goal at Apple Creek Place is to maintain a minimum 1:10 ratio in our communities at all times, Each building will always have one awake staff member present 24/7.
Our staff has bi-monthly staff meetings as well as regular in-services for continuing education. We also utilize virtual training for topics throughout the year, and have a reimbursement program for approved continuing education programs pertaining to advancement of our staff.
Apple Creek Place Assisted Living is pet-friendly, accepting cats and small dogs (breed restrictions may apply). Our property has plenty of green space surrounding the buildings, as well as an enclosed patio, for dogs
All residents are issued pendants when they move in. Entrances/exits utilize Stanley security systems for resident safety.
All of our buildings have secured entries, requiring key card or passcodes to enter/exit the building. We are also alerted anytime our main entries are opened.
Apple Creek Place employs a full-time Life Enrichment Coordinator that provides meaningful activities to residents based on their needs and interests. Activities include: morning stretches, memory games, bean bag toss, group physical therapy, happy hour, and music therapy. Staff also engage memory care residents one-on-one to help redirect wandering, and cope with confusion and sun downing.
Apple Creek Place has a full-time culinary coordinator on staff with access to a nutritionist/dietician to consult on individual resident needs. Three meals a day are included: Breakfast is made to order, while lunch and dinner has a pre-set "Special" of the day, with the option to choose from our Always Available menu whenever they'd like. Snacks are included and available 24/7.
Apple Creek Place is located on the north side of Appleton, nestled between a residential subdivision and a farm (complete with llamas and donkeys!). While there aren't necessarily attractions for residents to walk to, there are ample sidewalks for those that wish to take walks when the weather is nice.
Our community is located at the entrance to a beautiful residential neighborhood, with a farm and fields surround the other sides.
At Apple Creek Place, our meals are served restaurant-style on bright colorful plates to help provide contrast to the food being served and stimulate appetites. We also have picture menus available to help residents express their choices when their voices or memories fail them.
Families and guardians are kept as informed as they'd like to be, based on the preferences of the residents or power of attorney. Some residents have very involved family members while others are less involved in the day-to-day life of their loved ones. A combination of phone calls, emails, and Facebook event listings are used to keep families informed.
In case of emergency, a resident's emergency contact or decision maker will be notified once the resident has received proper care and attention to stabilize them.
Apple Creek Place is a wonderful community of three buildings, each with a maximum capacity of 22 residents. This allows residents to get to know each other and form meaningful relationships with their neighbors. Residents range in age from 55 to 90+ with varying interests. Some enjoy participating in group activities or watching movies and TV together, while others prefer quieter areas and activities, like reading by the fireplace or visiting in the sunroom. We also work to help residents fulfill their dreams and desires through our "Dare to Dream" program, that coordinates activities and events based on things a resident has always wanted to do or something they used to do that they'd like to do again.
Apple Creek Place provides transportation to medical appointments as well as coordinated group activities in the local community. We utilize Fox Valley Cab, as well as our own 14-passenger wheelchair/accessible bus.
