Our committment to person-centered care extends to our culinary experience at The Koselig House Assisted Living and Memory Care. Our Chef works with a Registered Dietician to select a menu that is both nutritious and delicious, but also reflects the personal likes of each residents. In fact, we collect information on food and beverage preferences from each resident and solicit special family recipes that make everyone's mouth water. When creating these masterpieces of flavor our Chef will often engage our residents to help in the process, which means that homemade cookies and deserts are a regular affair in our dining room. We also work to adjust our menu to meet the medical needs of our residents and offer finger foods, pureed diet, mechanical soft diet, and others. Our goal is to ensure that every residents has an exceptional dining experience and invite our families to join us too!

The Koselig House Assisted Living and Memory Care is honored to serve seniors 55 years of age and older who are in need of assistance with general daily activities and tasks, medication management, mobility or other needs. In addition, specialized supports are offered to those with a diagnosis of dementia related to Alzheimer's disease, Lewy Body, Frontotemporal, Vascular, Wernicke-Korsakoff or another form of dementia. Regardless of why someone chooses to join our Koselig family, all residents enjoy delicious meals, snacks, and beverages prepared by our Chef using a nutritious menu directed by a Registered Dietician. In addition, we offer a robust calendar of activities tailored to the interests, hobbies, and past history of each resident. As a result, our residents spend the majority of their time engaged in smiles, conversation and moments that bring forth joy every day. Residents are also empowered to be active in the community by attending concerts, local restaurants and more!
Unfortunately, 6 out of 10 people with dementia will wander at some point in their disease process. This is why we have purposefully designed our home to offer internal and external areas to allow for this type of behavioral symptom. This includes wide indoor walking paths, socialization and engagement spaces that ensure proper oversight and outdoor areas to refocus residents with more appropriate activeness. We also recognize that wandering is often a sign of being bored or lonely, which is why we encourage residents and family members to decorate their loved one's room with personalized pictures and items from home. It is also why we collect an extensive life history profile upon admission, so that we can learn information about each resident and tailor an activity and conversation plan to directly reflect their personal history, likes, and interests. In doing so we can create opportunities for personalized engagement that focuses them on smiles versus steps.
Our person-centered approach to care ensure that our residents are supported at all times, with additional assistance for those with memory loss. An example of this would be our enhanced culinary experience for those with Alzheimer's disease or another form of dementia. During a meal our team partners with these residents to encourage socialization, monitor intake, and observe for any changes in condition. They also work to ensure meals are provided in a way that supports the individual physical needs and dignity of our residents by ensuring their meat is cut up within the kitchen versus at the table. That the silverware given is adjusted for hand tremors with a larger or weighted spoon or that the food is sectioned in bowls versus a plate to help with spillage issues. We recognize that mealtime is an important part of the day and that every resident deserves to have a high quality, delicious culinary experience.
At The Koselig House we provide a full spectrum of daily services and more than just a comfortable place to live. Our residents are encouraged to be active, eat well, participate socially, engage mentally, and live a purposeful life. Residents of The Koselig House can experience several life enriching activities per day which center around Exercise, Spirituality, Music and the Arts, Sensory Therapy, Cognitive Stimulation, and Life Skills. Each is tailored to the resident to promote independence and choice.
In addition, we follow a person-centered approach which means means our exceptional Team goes above and beyond to tailor care and engagement to match the interests, abilities, history, and personality of each individual resident. By doing this, we know that it helps ensure that those in our care are enabled to take part in the things they enjoy, and to live the highest quality of life possible.
We encourage our residents and their families and friends to participate in all activities of the home and alert them to these opportunities by posting them within the home in various locations, sending a newsletter to those who request it, in personalized emails or mailed invites, and via our Facebook and LinkedIn pages.
Due to HIPAA requirements, depending on the status of the resident care needs and issues will only be discussed with those who are legally able to receive them or who have been given permission by the resident or their activated power of attorney for healthcare or court appointed guardian. The communications may occur in-person, vai phone, email, or in whatever manner is most appropriate to the receiver. Typically they occur in real-time, as the need arises, is requested, or at least quarterly. Our goal at The Koselig House it provide an open door policy for residents and families, while maintaining the rights and privacy of those for whom we care.
At this time, The Koselig House is not able to accept dogs or cats, but this may change based on the capability of the owner and preference of the house. Therefore, please reach out to determine if arrangements could be made to help ease this transition. If nothing, else we encourage pet therapy and visits, so perhaps a family could house the animal and bring for visits as able.

When a resident is engaged during the day or night our team is empowered to spend time conversing with them and sharing a joyful moment. We recognize that it is in these moments that true care and love is expressed and is most needed, however one must have the time and respect to take the time to allow it to flourish.

We are proud to work with all local medical providers in the DeForest and surrounding communities. If a resident prefers to go into Madison for care, we will work with that physician to ensure any orders or needs are met. In addition, we have a wonderful relationship with hospital and clinics closer to home too.
In fact, we offer a variety optional health services for our residents right at The Koselig House Assisted Living and Memory Care. These include diagnostic services, podiatry, palliative care, hospice, home health, physical therapy, speech therapy, lab work, incontinence products, and pharmacy supports.
The Koselig House Assisted Living and Memory Care was purposefully designed to meet the needs of our residents, which means that you will find our home to be one-story, with private residents rooms and bathrooms off-set from main living spaces. These living spaces are open concept with a variety of socialization, engagement, and dining areas located throughout our home. Our hallways have been widened with enhanced side rails to assist with mobility and independence. The home is secured through an egress door system that alarms should a resident leave the home without proper supervision. In addition, our back courtyard is fully fenced and provides additional spaces for seating, gardening, and space to enjoy the outdoors. Residents are also assisted using an audible safety system that alerts the caregivers to any issues that may arise. Lastly, our care team is trained to understand the aging and dementia process and ensure that those in our care our treated with dignity and respect.

Assisted Living residents at The Koselig House may still have access to their cars and are offered free parking just outside of our home. In addition, we contract with a local transportation provider to ensure those who are fully mobile and those in wheelchairs have access to the community for medical appointments or social outings.
A free assessment is provided to each resident prior to admission to The Koselig House Assisted Living and Memory Care of DeForest. This assessment, along with our life history profile is used to create an individualized care plan for each resident. This plan of care is then updated on a regular basis depending on the needs of the residents. This could mean that updates are done multiple times a day, week, or month. If there is a change of condition, a fall or some other medical need that arises the resident would be reassessed and this information communicated to the resident or appropriate legal party. At a minimum each resident is reassessed quarterly to ensure that all of the medical, physical, mental, emotional, and spiritual needs are being met.
