We are not directly affiliated with any hospitals but work closely with many hospitals in the area and have a Kaiser Dementia Physician come onsite to care for Kaiser patients on a regular basis.
We schedule a minimum of 1 med-tech and 3 caregivers for Day & Swing, and have 2 staff members overnight. Additionally we have nurses onsite 16 hours a day.

We have a designated Memory Care Director onsite 5 days a week that communicates to families of changes in health, schedules regular care conferences and coordinates support groups. In addition to this role, with licensed nurses onsite 16 hours a day, nurses will work be the first responders if anything were to happen and require immediate response.
We are located right between residential communities and in walking distance to an elementary school, library and shopping centers.
We can offer either stand by assistance with prompting or hands on assistance using the hand over hand technique.

Our nurses who are onsite 16 hours a day assess residents. Assessments are done at time of move in, 30 days after move in and at least 1x a year after that. We always reassess if resident displays a change in their health and cognitive state.
