FAQ About Manor Lake Assisted Living & Memory Care
Person centered care, approach and behavior management, quality dementia care, improving communication, dementia and hospice care, progressions of dementia.
We have 5 day a week transportation via Brookdale van or bus and an amazing driver that is loved by all.
Pull Call lights in apts.. and common areas. Each resident has a pendant to push for when care is needed or in an emergency, safety checks on residents.
Brookdale Folsom is located in a residential neighborhood with medical offices and hospital close by. There are doctors’ offices and other medical types facilities nearby.
Our residents are active from 75-103 years of age. Their favorite activities are B-Fit exercise, Happy Hour and Bingo.
We are right next door to Mercy Folsom and work with all medical facilities including Kaiser an the VA.
Our nursing team in conjunction with their primary care doctors, assessments every 6 months and change of condition.
Staff are trained monthly on a variety of topics, including but not limited to fall prevention, resident rights, abuse and neglect, approach and person centered care, fire drills, medication management, hospice care.
Care conferences held with nurse, executive director and memory care director, care plans.
Our staffing is determined directly from our care plans. The more care people require the more staff are allotted. The care plans are personalized and staffing will flex to meet those individual needs.
Monthly newsletters are mailed to families. We have a bulletin in AL and CB to show upcoming activities and events. Care conferences are every 180 days and any time there is a change of condition. The ED and Nurse have an open door policy. They are available for families and residents.

Both cats and dogs are allowed in both of our neighborhoods. There is a “dog run” in AL and the courtyard in the Memory care for them to play.

We offer a wide variety of great tasting meals that can be tailored to suit each resident’s needs as well as Brookdale Seasonings to add that extra punch of flavor. Our chef conducts menu chats with residents to help provide meals they enjoy. Specialty Cuisine/Diets.
B-Fit, Brain Fit, scenic drives, outings, happy hour, entertainers, life 360, Bible study, church services.
Our exterior doors are locked at night. Staff will identify visitors before entering. All visitors must check in at the front desk. We have 24 hour awake staff to provide care and security 24 hours a day.
Cueing, hand over hand and full assist feeding if required.
We have a walking trail across the street that leads to local shopping ie: Target, Trader Joe's, Home Goods, and multiple eateries.
